What is the difference between All-on-4 and All-on-6 in practice?

In the case of All-on-4 restorations, four dental implants are placed at specific angles. After the healing period, these implants securely anchor the entire prosthetic restoration. For these four implants to provide sufficient stability, they must be of a larger size.

In cases where bone loss is so advanced that there is insufficient bone quantity for the placement of implants with the proper thickness and length, smaller screws need to be used during the implant procedure. In this case, two additional dental implants are placed to ensure stability.

Despite severe bone loss, our patient received a fixed, stable denture with the All-on-4 solution for the upper jaw and the All-on-6 solution for the lower jaw, regaining the joy of chewing once again.

The procedure for All-on-4 and All-on-6 restorations

  1. Diagnosis and developing a treatment plan
    The All-on-4 or All-on-6 treatment always begins with a consultation with an oral surgeon. During the consultation, a thorough oral examination is conducted, along with a 3D CT scan. This imaging is essential for any implant procedure, providing the surgeon with a comprehensive understanding of the available bone structure.
  2. Surgery – implant placement
    During the All-on-4 or All-on-6 procedure, four or six implants are placed simultaneously into the jawbone under local anaesthesia. Following the placement, an impression is taken to design the temporary prosthesis. The temporary fixed denture, which is screw-retained, is ready five to eight business days after the surgery, eliminating the need for a removable denture.
  3. Check-ups and adjustments
    The temporary All-on-4 or All-on-6 restoration is checked by the dentist one month after placement. Adjustments may be made if necessary to ensure the patient’s comfort during the healing period.
  4. Healing period – osseointegration and final prosthesis
    The healing time required for the implants to integrate with the bone varies but typically takes around four months. Once healing is complete, the implants can support the final prosthesis. Either the temporary denture is finalised, or a new permanent denture is fabricated based on its design, a process that takes just a few business days.

The oral examination and 3-dimensional CT scan taken on site confirmed that the existing teeth could not be preserved, so the oral surgeon decided to extract them and recommended the All-on-4 solution to our patient, i.e. the placement of 4 implants in the lower and upper jawbones, on which a fixed, fixed denture can be placed after healing.
